Alyssa Ashcroft es la criatura que te caza en la demo. Tras su muerte durante el prólogo, el villano Victor Yurichenko se hace con su cuerpo.
Leon S. Kennedy formará parte del juego de dos formas distintas: primero en busca de Víctor, que está siendo investigado por el gobierno estadounidense debido a su conexión con Operación Elpis, que busca grupos secretos relacionados con el pasado de Umbrella y Raccoon City. La segunda parte es más adelante, yendo al rescate de Grace Ahscroft.
Yoko Suzuki y Claire Redfield ambas tienen roles menores en el juego. Durante la historia, Jill Valentine y Sherry Birkin son ambas mencionadas como supervivientes importantes del incidente de Raccoon City.
El juego cuenta con aproximadamente seis boses principales: tres para Leon y otros tres para Grace.
Volveremos a enfrentarnos con criaturas como los Lickers, G-Types y Drain Deimos con mutaciones menores y peor aspecto.
El virus del juego es el Elanthropus, y está sacado de la investigación de Victor Yurichenko sobre el moho y el virus progenitor. Este virus mantiene la conciencia del sujeto en algunos infectados, y causa transformaciones exageradas en otros.
El juego dura aproximadamente 18 horas, y cuenta con un modo extra parecido a los Mercenarios.
The Connections juega un papel sorprendente en el juego, ayudando a Leon a encontrar a Grace y parar un brote importante en Rhodes Hill.
